In a discussion with HT Lifestyle, Dr. Bipeenchandra Bhamre, a Consultant Cardiac Surgeon at Sir HN Reliance Foundation Hospital and Research Centre in Mumbai, explained, “Bypass surgery for the heart, clinically referred to as coronary artery bypass grafting (CABG), is a surgical intervention aimed at restoring blood flow to the heart when its arteries are narrowed or obstructed. This condition, often caused by atherosclerosis, can restrict the delivery of oxygen-rich blood to vital areas of the heart muscle, potentially resulting in chest pain or more serious cardiac complications. Consequently, bypass surgery may be necessary.”

Dr. Bhamre emphasized that the significance of bypass surgery extends beyond its immediate objective of improving blood circulation; it also has the potential to significantly enhance patients' overall health. He noted, “Many patients experience a marked improvement in their quality of life after surgery, allowing them to resume their activities without difficulty. Furthermore, advancements in surgical techniques and technologies have led to the availability of minimally invasive procedures that are both safe and effective, thereby shortening the recovery time. Nevertheless, it is essential to adhere to the guidelines provided by your cardiac surgeon to maintain your health and achieve successful outcomes.”

Precautions after the bypass surgery

Dr. Bipeenchandra Bhamre stated, “The recovery period following bypass surgery can be likened to a month of 'tapascharya.' The first week will be spent in the hospital, including three days in the Intensive Care Unit (ICU). As you begin to recover in the ICU, the ventilator and other monitoring tubes will be gradually removed. Many ICUs are implementing fast-track protocols to facilitate patient recovery. Ambulation will commence 24 hours post-surgery, starting with sitting at the edge of the bed, progressing to a chair, and then walking after two days. It is normal to experience pain due to fresh stitches, which typically decreases by 50% after two days and further reduces to 10% by day four.”

He suggested -

1. Once you begin walking, start with sessions of 5 to 7 minutes, gradually increasing the duration as your strength improves. By the end of one month, you should aim to walk for 40 minutes a day, divided into two 20-minute sessions.

2. Engaging in deep breathing exercises is crucial and should be continued for two months following the surgery.

3. During your hospital stay, you will be cared for by the doctor, nursing staff, physiotherapists, and other team members. Upon discharge, both the patient and their family will receive instructions to adhere to the recovery plan. It is important to note that recovery can vary; some days you may feel energetic, while on others, rest will be necessary.

4. It is essential to remain at home to minimize the risk of exposure to external infections.

5. Visitor restrictions should be observed, maintaining a distance of six feet during conversations.

6. Strict adherence to cleanliness, including hand hygiene and daily skin care through scrub baths or water baths as directed by your surgical team, is necessary.

7. Bending forward or sleeping on your sides or stomach should be avoided for 4 to 6 weeks until the incision has fully healed.

8. Within your home, you can remain active by walking and performing regular stretching and breathing exercises as instructed by your team. Remember, human healing is enhanced by rest, and home is the ideal environment for relaxation and rejuvenation.

9. During your recovery from bypass surgery, it is generally recommended to consume simple, home-cooked meals that are easy to digest and provide sufficient energy for healing. Incorporating proteins such as milk, paneer, soy, various pulses, curd, and buttermilk, along with green vegetables, chapati, and rice, will adequately support your recovery needs.

10. Typically, the surgical wound will require dressing for a period of 2 to 3 weeks until all sutures have been removed.

11. It is advisable to cease smoking and the use of any tobacco products.

12. It is important to refrain from alcohol consumption during the recovery phase.

13. Prioritize a nutritious diet by including a diverse range of fruits, vegetables, and whole grains, while limiting your intake of sugar, salt, and saturated fats.

14. It is crucial to manage your body weight, as excess weight gain post-surgery may occur due to fluid retention. Therefore, it is necessary to limit water intake for several weeks during recovery.

15. Engage in regular physical activity, as exercise is vital for managing diabetes, high cholesterol, and high blood pressure, all of which are risk factors for heart disease. Aim for at least 30 minutes of light exercise, contingent upon your doctor’s approval.

16. Manage stress through mindfulness techniques such as yoga and meditation, and consider participating in support groups. If you experience feelings of anxiety or depression, consult your doctor for potential coping strategies.
