The human body is known to contain over a hundred distinct hormones. These hormones play a crucial role in facilitating communication between various organs, skin, muscles, and other tissues through the bloodstream, thereby assisting the body in coordinating numerous physiological processes.
The majority of hormone-producing and releasing tissues, primarily glands, are integral components of the endocrine system. In a discussion with Lifestyle, Dr. Varini N, Senior Consultant - Obstetrician and Gynaecologist at Milann Fertility Center in Bengaluru, stated, “Hormones regulate a wide array of bodily functions, including metabolism, homeostasis (the maintenance of internal balance), blood pressure and blood sugar control, fluid and electrolyte balance, body temperature, growth and development, sexual function, reproduction, sleep-wake cycles, and mood.”
She also noted, “Even a small quantity of hormones can exert a significant influence. Consequently, minor fluctuations in hormone levels can lead to substantial effects on the body, potentially resulting in medical conditions that require intervention. Conversely, hormonal imbalances or prolonged exposure to certain hormones may increase the risk of developing reproductive cancers. Hormones such as progesterone and estrogen can elevate the likelihood of ovarian, endometrial, and breast cancers, among other reproductive malignancies. Hormones affect cell division, which can lead to the accumulation of genetic mutations and the onset of cancer.”

Factors that increase the likelihood of developing reproductive cancer include:
1. Reproductive history: An increased risk of breast cancer may be associated with early onset of menstruation, delayed menopause, and a history of not having children.
2. Hormone replacement therapy (HRT): The combination of estrogen and progestin in HRT can contribute to a heightened risk of breast cancer.
3. Obesity: Excessive body weight may elevate the risk of both endometrial and breast cancers.
4. Endometriosis: There is a significant correlation between endometriosis and the occurrence of Type I ovarian cancers.
The impact of hormones on reproductive cancers
The health of the reproductive system is significantly influenced by hormones such as testosterone, progesterone, and estrogen. However, prolonged or excessive exposure to these hormones has been associated with various malignancies:
1. Breast Cancer: The development of breast cancer is closely linked to progesterone and estrogen. Women who engage in long-term hormone replacement therapy (HRT), experience early onset of menstruation, or undergo late menopause face an increased risk. Hormone-sensitive breast cancer cells may grow more aggressively due to the presence of these hormones.
2. Ovarian Cancer: Studies suggest that hormonal factors, including the frequency of ovulatory cycles, may affect a woman's likelihood of developing ovarian cancer. Conditions that disrupt hormonal balance, such as polycystic ovarian syndrome (PCOS), may also play a role.
3. Endometrial Cancer: Unopposed estrogen exposure, which occurs when progesterone levels are insufficient to counterbalance estrogen, is a primary risk factor for endometrial cancer. This condition is often observed in obese women, as adipose tissue produces estrogen. A similar impact is noted in cases of PCOS.
4. Prostate Cancer: In men, both dihydrotestosterone (DHT), a more potent form of testosterone, and testosterone itself are associated with prostate cancer. Elevated levels of these hormones may stimulate the growth of malignant prostate cells.
Mechanisms linking hormones to cancer
Hormones influence cancer risk through various mechanisms:
1. Cell Proliferation: In hormone-sensitive tissues, hormones such as testosterone and estrogen stimulate cell division. This increased rate of cell division can lead to mutations, thereby elevating the likelihood of cancer development.
2. DNA Damage: Certain hormones can produce metabolites that induce DNA damage and oxidative stress, contributing to the onset of cancer.
3. Receptor Activation: The binding of hormones to specific cell receptors triggers pathways that may lead to uncontrolled cell proliferation.
4. Immune Suppression: Hormonal imbalances can hinder the immune system's ability to detect and eliminate abnormal cells, facilitating the proliferation of cancerous cells.
Risk factors
Dr. Varini N stated that various factors, including age, family history, lifestyle choices, and medical history, can exacerbate the hormonal connection to reproductive cancers. Individuals of advanced age are at a higher risk of developing hormonal malignancies due to their prolonged exposure to hormones. Additionally, genetic predispositions, such as mutations in the BRCA1 or BRCA2 genes, can further intensify the effects of hormone exposure.

She stated that conditions such as PCOS, endometriosis, and prolonged use of hormone therapy heighten vulnerability. Birth control pills contain estrogen and progesterone, which are female sex hormones, and are utilized to manage fertility. These hormones may lower the risk of endometrial and ovarian cancers. Conversely, they slightly increase the risk of cervical and breast cancers.
Prevention and early detection
Increasing awareness and providing education regarding the early signs and symptoms represent the most effective and widely adopted preventive approach. Dr. Varini N stated, “Maintaining balanced hormone levels can be achieved through a nutritious diet, regular physical activity, and sustaining a healthy weight. Routine screenings, including PSA tests, pelvic examinations, and mammograms, are essential for early detection. For patients identified as high-risk, preventive measures such as selective estrogen receptor modulators (SERMs) or prophylactic surgery may be considered. Additionally, the judicious use of oral contraceptives and hormone replacement therapies, under medical guidance, can help mitigate risk.”
