The research: Alcohol, cholesterol levels, and a database of 58,000 individuals
A study monitored almost 58,000 adults in Japan for a year, looking at their medical records to see how their cholesterol levels changed. The researchers found something intriguing: when people who didn’t drink started consuming alcohol, their LDL (the bad cholesterol) went down, and their HDL (the good cholesterol) went up.HDL levels actually got so much better that they surpassed the effects of certain cholesterol meds. On the flip side, people who stopped drinking alcohol experienced an increase in LDL levels and a decrease in HDL levels.
The research revealed a clear link between alcohol consumption and changes in cholesterol levels.:
- Light drinkers, who consume up to 1.5 drinks daily, experienced a small decrease in LDL by 0.85 mg/dL and a slight increase in HDL by 0.58 mg/dL.
- Those who drink moderately, between 1.5 to 3 drinks a day, had a more noticeable drop in LDL of 4.4 mg/dL and a significant rise in HDL of 2.49 mg/dL.
- Heavy drinkers, consuming over 3 drinks a day, showed the most dramatic changes, with LDL decreasing by 7.44 mg/dL and HDL increasing by 6.12 mg/dL.
On the other hand, people who stopped drinking saw the opposite effect:
- Light drinkers who stopped drinking experienced an increase in LDL by 1.10 mg/dL and a decrease in HDL by 1.25 mg/dL.
- Moderate drinkers who quit saw their LDL go up by 3.71 mg/dL, while HDL dropped by 3.35 mg/dL.
- Heavy drinkers who gave up alcohol had their LDL rise by 6.53 mg/dL and HDL fall by 5.65 mg/dL.
How does this impact your health?
Just so you know, the ideal LDL level is under 100 mg/dL, and HDL should be at least 60 mg/dL. Keeping LDL low helps lower your heart disease risk, while having higher HDL provides some heart health perks.
Even minor tweaks in cholesterol can make a big difference—raising LDL by just 5 mg/dL can bump up the risk of heart issues by 2-3%. The research took into account various factors like age, gender, BMI, exercise routines, and smoking habits. The findings were steady across all types of alcohol, whether it’s wine, beer, sake, or spirits.

This study adds to the conversation around alcohol and its potential heart benefits, but it’s definitely not an excuse to start drinking heavily. Earlier research has shown that moderate alcohol consumption might lower the chances of heart attacks and strokes, but it also raises the risk of certain cancers like breast, liver, and colorectal.
So, what should you take away from this? If you already enjoy a drink, keep it moderate. If you don’t drink, this study doesn’t mean you should start just to improve your cholesterol.
